Serious Incident

An incident involving circumstances indicating that there was a high probability of an accident, such as overrunning,emergency evacuation, fire or smoke inside the cabin and abnormal depressurization, encountering abnormal weather conditions, etc, as classified by MLIT.

Go Around by Japan Air Commuter Flight 3626
Outline On May 10, 2011, Japan Air Commuter Flight 3626 (from Miyazaki to Fukuoka) received permission for landing from ATC and was approaching Fukuoka Airport when ANA Flight 487, which was scheduled to take-off from the same runway, entered the runway with permission for take-off from ATC. The JAC aircraft made sure of permission for landing with ATC, and the aircraft performed Go Around under ATC instruction. None of the passengers and crews were injured. This case was classified as a Serious Incident by MLIT.
Investigation into the cause Japan Transport Safety Board, MLIT is investigationg the cause of the accident.Japan Air Commuter is providing full cooperation with the investigations and implementing countermeasures.
Safety Trouble

In accordance with Laws enforced on October 1, 2006 (Civil Aeronautics Law, Article 111-4 and relevant Enforcement Regulations Article 221-2, Item 3 and 4), it is mandatory to report to MLIT of the following occurrences. These troubles may trigger an accident if they accumulate, but they do not immediately lead to an aircraft accident.

Classification of safety troubles and examples
Aircraft damage caused by lightning strike, bird strike, etc.
System trouble
(e.g. trouble with engine, communication or electrical systems)
Trouble with instruments activated in an emergency
(e.g. malfunction of fire/smoke detector)
Operations exceeding standards
(e.g. exceeding the speed limitations)
Sudden operation due to instructions by instruments
(e.g. operation based on instruction of TCAS (Traffic Alert & Collision Avoidance System))
